SERVICES

• Full mental health evaluation including diagnostic lab tests as necessary 

• Psychotherapy (talk therapy) or collaboration with your psychotherapist if you already have one

•  Prescription medications if necessary

• Collaboration with your other treating physicians

• Psychoeducation targeting prevention of future symptoms

• Referrals to community resources and support as appropriate

The initial appointment is 1.5 hrs and the follow up appointments are 45-50 minutes. You may print the forms below to fill in ahead of time or complete them at your first appointment.  Both telehealth and in person appointments are available. 

Fees are payable at the time of service by check, cash, or credit card.  I am not in network with any insurance companies but I provide super-bills that you can submit to your health insurance company for reimbursement (only if you have PPO insurance). If you would like to ask your  your insurance company what your share of cost would be, they will need to know the procedure codes I use. The codes I use most frequently are:
